I don't think I've seen a single article that says why. Even though odds are it's probably one of the more common issues (like colic, paddock accident, heart attack, or anaphylactic shock to what should've been a routine injection), it'd be nice to hear it. Did we ever find out why Carson City died? I remember looking for the autopsy results and never saw any.

If they aren't saying, it's probably a human reason. Like yet another injection gone bad.
