You might want to try putting with your eyes closed, or while looking at the hole. I think Johnny Miller did both of these things, before giving up the game completely

trick I use if putting gets off, to get it back..... during practice, line up the put, visualize it, then make the stroke with your eyes closed (tip from pro I learned putting from). will show you any tendencies you are making (push, pull, etc). then get 2 blocks of wood and set on ground with minimal clearance to putter (tip from Nicklaus's Golf My Way). make a ton of putting strokes without touching the blocks. that will get your consistancy back and give you a ton of confidence, as you should basically make every single putt with the blocks (putt has to be on line every single time).

Works too. Missed winning best putter award on high school golf team by 1 shot (avg'd about 28 for the year). Don't play enough to keep it there now (would like to be able to play more and get it back to there), but that drill always gets me straightened out for a short period of time still, use it if tournament comes up that I'm playing in.
