I don't think they left any loose ends. The ending is pretty self explanatory. The guy staring at them was a red herring.

That was Chase's intent. Build the suspense and make you forget that a deal was made with Little Carmine and Butchie, Phil was murdered, and the real end to Tony...Carlo flippin'. So it really wasn't happily ever after...just not the blood and guts finale most fans crave.

So in the end, Tony gets whacked.
That's why the screen went black and the music stopped.
As he told Bobby Bacala in the first episode in Season 6, Part II, you never see it coming.
We see it coming with the guy going to the bathroom and the two gentlemen entering the restaurant.
Meadow is saved because she can't parallel park the car.
Shocking.
Jolting.
Took me a few minutes to figure out what happened (actually my stepson had to explain it to me).
